Welcome to Aspire. You can save yourself some time by printing off our intake paperwork packet in the link below. Please open it up and fill it out and bring to your first visit. Please bring your insurance card and photo ID. If you bring a copy of those items; or we can copy them in-office, you will not have to fill out the insurance information piece in the intake packet. Please be aware if you need to make a copay or coinsurance payment at the time of your first visit and what that amount is. Please be advised that the paperwork must be signed by a parent or legal guardian if you're bringing a child to the first visit and he or she is receiving our care. If you're coming to see us for couples counseling, we prefer that the person who made the initial contact fill out the paperwork: for insurance purposes there can only be one insured name, not a couple.
